Job Summary:
Our client is actively seeking dedicated and compassionate full-time Family Nurse Practitioners (FNPs) to join our esteemed Primary Care practices located in Dundee, Seneca Falls, and Penn Yan. As a key member of our healthcare team, you will play an essential role in providing high-quality, patient-centered care to individuals and families across these communities. We are looking for practitioners who are not only skilled and experienced but also committed to building long-term, trusting relationships with their patients. This role is ideal for someone who values continuity of care and wants to make a lasting impact in their patients’ lives.
Key Responsibilities:
- Patient Care: Provide comprehensive primary care services, including diagnosing, treating, and managing acute and chronic health conditions. Emphasize preventive care, health education, and wellness strategies to help patients achieve optimal health outcomes.
- Long-Term Relationships: Foster strong, ongoing relationships with patients, becoming a trusted healthcare partner for individuals and families. Focus on continuity of care, ensuring patients receive consistent, personalized treatment throughout their lives.
- Collaborative Practice: Work closely with physicians, specialists, nurses, and other healthcare professionals to deliver integrated care. Participate in team meetings and contribute to the development of care plans that address the unique needs of each patient.
- Patient Education: Educate patients and their families about health conditions, treatment options, and preventive care strategies. Empower patients to take an active role in managing their health and well-being.
- Documentation: Maintain accurate and up-to-date medical records, ensuring compliance with all regulatory requirements. Document patient interactions, treatment plans, and progress notes in a timely and thorough manner.
- Community Engagement: Act as an ambassador for Finger Lakes Health within the local community. Participate in community outreach programs, health fairs, and educational seminars to promote health and wellness in the region.
Qualifications:
- Board Certification: Must have current and active Board Certification as a Family Nurse Practitioner.
- NYS License: Must hold a valid and unrestricted New York State (NYS) License to practice as a Family Nurse Practitioner.
- Experience: Prior experience in a primary care setting is preferred, with a demonstrated ability to manage a diverse patient population across the lifespan.
- Clinical Skills: Proficiency in conducting physical exams, ordering and interpreting diagnostic tests, developing treatment plans, and prescribing medications.
- Communication: Excellent interpersonal and communication skills, with a strong ability to connect with patients, families, and healthcare team members.
- Compassion and Commitment: A genuine passion for patient care, with a focus on providing kind, empathetic, and consistent care. A commitment to building and maintaining long-term patient relationships is essential.
