Job Summary:
The Licensed Vocational Nurse/Licensed Psychiatric Technician (LVN/LPT) at the Sobering Center is responsible for providing direct patient care services, including medication management, administration, counseling, and health maintenance. This role involves evaluating patient needs, assisting in treatment plan implementation, monitoring medication intake, maintaining medical supplies, and documenting patient care and progress. This facility will be a 24/7 operation and requires a flexible work schedule including nights, weekeneds, and holidays.
Responsibilities:
1. Plan and deliver nursing assessments and interventions based on medical and psychiatric diagnoses.
2. Administer medications as ordered, assess effectiveness, and monitor for side effects.
3. Conduct pre-admission triage to determine medical/nursing appropriateness of admission.
4. Monitor patient medication intake according to policies and standards.
5. Report significant changes in patient condition promptly.
6. Document nursing goals, interventions, and outcomes in patient charts.
7. Collaborate as an active member of the treatment care planning team.
8. Respond to medical emergencies.
9. Utilize necessary equipment for patient care delivery, such as glucometer, AED, bag/mask, and oxygen.
10. Conduct Blood Draws as per treating provider orders (additional certification required).
11. Attend required trainings.
12. Perform other job-related tasks as assigned.
Qualifications:
- Graduate of an accredited LVN or LPT school or college and licensed to practice in California.
- Two (2) years of recent acute care experience preferred; Behavioral/Chemical Dependency Setting experience required.
- Excellent written and verbal communication skills.
Licenses/Certifications:
1. Current California LVN/LPT License.
2. Current CPR Card.
