Community Service Worker

Lake County (Madison ICAP Office)

CSW Duties and Responsibilities (All)

  1. Remain knowledgeable on assistance programs for low-income such as Social Security, Medicare, Medicaid, Food Stamps, Supplemental Security Income, Housing Programs, etc.
  2. Identify, interview, and assist persons in need of agency services, under direction of professional staff.
  3. Refer clients to community resources and other organizations. Develop and maintain service networks with collaborating agencies, providers, and other entities to include, but not limited to, the department of social services, police station, county health nurse, clinics, etc.
  4. Take applications for agency services and programs. Assist in preparing applications for non-agency programs. Programs would include State Tax Refunds, Food Pantries, Weatherization, Emergency Food/Fuel, Head Start, Real Estate Tax Freeze, etc.
  5. Provide information on basic life skills. Perform Protective Payee Services for participants as needed.
  6. Coordinate seasonal programs to assist with annual programs such as the angel tree, swim passes, food baskets, clothing drives, etc.
  7. Prepare tax returns for Volunteer Income Tax Assistance program.
  8. Oversee operation of the thrift store if applicable.
  9. Assist with local disaster coordination and collaboration of relief.
  10. Supervise the Food Pantry to include distributing food to participants, bookkeeping, and inventory, etc., if applicable.
  11. Represent low-income individuals in situations of potential or actual exploitations, neglect, or abuse to protect their rights and entitlements.
  12. Promote positive public relations in all areas of the Community Services Program. Seek out opportunities to educate the public regarding the Program. Be available to speak at service groups when given those opportunities.

60's Plus Site Manager Duties and Responsibilities (If Applicable)

Site Responsibilities
  1. Oversee general operation of entire Nutrition Site(s). Visit satellite site(s) on a regular basis. Monitor service, participant satisfaction and food quality at these sites.
  2. Accept and record daily meal reservations for congregate and home-delivered meals when applicable.
  3. Set up home delivered routes for drivers when applicable.
  4. Coordinate daily meal count with Cook, Café, or Hospital in a timely manner.
  5. Ensure menu compliance as approved by the Nutrition Specialist.
  6. Monitor food quality, temperature, taste, and appearance. Evaluate sanitation practices according to Health Department regulations. Make suggestions for methods of food prep, which would enhance taste, and appearance of meals.
  7. Record participation and donation totals daily and deposit daily donations. Submit report to Director.
  8. Order site supplies according to program policy. Work with kitchen staff (where applicable) to obtain repair services for equipment. Submit bills for site expenses to the Director.
  9. Assist with the Cook, Kitchen Assistant, Dishwasher, and/or Janitor duties as needed.
  10. Enlist adequate community volunteers to ensure enough assistance in the completion of daily duties.
Administrative Duties
  1. Complete assessments and eligibility determinations for prospective and established participants. Send appropriate completed forms to Director.
  2. Conduct outreach efforts to potential program participants.
  3. Oversee distribution of nutrition education each month.
  4. Organize, publicize, and implement monthly site activities, which provide enjoyment, diversion, education, and relaxation to the participants. Utilize a variety of community resources in planning these events.
  5. Participate in organization and implementation of fundraising activities for cash match.
  6. Enter data from NAPIS assessments into SAMS database as prescribed by specialized training.
Supervisory Responsibilities
  1. Directly supervises employees and volunteers in the 60's Plus Dining Program when applicable.
  2. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws when applicable.
  3. Responsibilities include assisting in the interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems when applicable.
  4. Manage conflicts and encourage a positive work environment when applicable.

CHW Duties and Responsibilities (If Applicable)
  1. Promote wellness by providing culturally and linguistically appropriate health or nutrition education.
  2. Provide informal counseling and social support.
  3. Building individual and community capacity by addressing social determinants of health
  4. Enhance the client's ability to effectively communicate with health care providers.
  5. Refer and connect clients and families to resources including food, housing, health insurance, health information, quality healthcare, and social services.
  6. Bring awareness of health barriers and needs from a community perspective.
  7. Educate clients about how to use health care and social services systems.
  8. Conduct outreach regarding enrollment in Children's Health Insurance Program, Medicare, and Medicaid
  9. Proactively identify and enroll eligible clients in Federal, State, local, private, or nonprofit health and human services programs
  10. Provide guidance regarding effective strategies to promote positive health behaviors and discourage risky behaviors.
  11. Mentor other CHWs (capacity building)
  12. Address the misinformation, fear, and stigma surrounding infectious diseases by providing timely and accurate information about transmission.
  13. Teach or promote methods and measures that have been proven effective in avoiding illness and/or lessening its effect.

  1. Encourage and promote the families' achievement of self-sufficiency.
  2. Attend meetings, training, and seminars as deemed appropriate and necessary by ICAP.
  3. Perform duties in a manner consistent with safe practices and policies.
  4. Maintain client confidentiality at all times.
  5. Submit all records/reports in an accurate and timely manner.
  6. Access and record participant and community resource information. Document all sessions and maintain accurate participant files.
  7. Maintain ongoing knowledge of existing and new agency programs for low-income persons.
  8. Submit weekly time sheets, activity reports and monthly mileage report to supervisor.
  9. Operate various equipment including but not limited to photocopier, telephone, fax machine, computer, calculator, scanner...etc.
  10. Promote ICAP Programs within the community by presenting a positive and cooperative image to participants, families, businesses, and other agencies.
  11. Network with other agencies to provide comprehensive services to individual/families with low income and the elderly such as social services, medical clinic, Extension, etc.
  12. Maintain open communication with the Director.
  13. Perform other duties as deemed appropriate and necessary.

Education and Experience

  1. High school diploma or general education degree (GED).
  2. One (1) year experience and/or training working with human services or equivalent combination of education and experience.
  3. Complete and maintain a Community Health Worker Certification through an approved SD DSS Medicaid CHW training program. (If Applicable)

Language Skills

  1. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
  2. Ability to write routine reports and correspondence.
  3. Ability to speak effectively before groups of clients or employees of an organization.

Mathematical Skills

  1. Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
  2. Ability to compute rate, ratio, and percentage.

Other Work-Related Abilities

  1. Ability to apply common sense understanding to carry out detailed but uninvolved written or verbal instructions.
  2. Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  3. Ability to work well with subordinates, public, and diversified populations.
  4. Ability to maintain confidentiality.
  5. Ability to be punctual and organized.

Certificates, Licenses, Examinations

  1. Valid South Dakota Drivers License.
  2. Other such examination as deemed appropriate and necessary by ICAP.
  3. Ability to be bonded.
  4. CHW Certification. (If Applicable)
