Tel Hai has a wonderful opportunity for a Social Worker to join our caring and dedicated team in our Meadows Health Care Center. This will be a part-time position working 24 hours per week with flexible day hours Monday-Friday. New Grads are welcome to apply

The Social Worker Intervenes to successfully address the residents needs and links social support, physical care and physical environment with resident needs and individuality. The Social Worker also assists residents in maintaining or improving their ability to manage their everyday physical, mental and psychological needs. They will act as an advocate for residents rights in all circumstances, promoting resident respect and dignity throughout their stay in the Meadows Health Care Center.
Additional Social Worker responsibilities include but are not limited to:
- Maintains contact with family (with residents permission) to report on changes in health, current goals, discharge planning and encouragement to participate in care planning.
- Identifies and seeks ways to support residents individual needs and preferences, customary routines, concerns and choices through the assessment and care planning process.
- Builds relationships between residents and team members and teaches team members how to understand and support residents individual needs.
- Demonstrates competency in assessment and range of care appropriate to the age of the resident cared for: Aging Population.
- Performs Discharge Planning which includes interdepartmental coordination and making all necessary outside agency referrals.
- Informs residents and/or their resident representative of insurance coverage and end of coverage (verbal notification and Medicare non-coverage letter).
- Monitors psychotropic medications and monitors the physician documentation of the need for these medications.
- Documents social service progress notes and maintains sufficient, appropriate and descriptive account of the progress and observations of each resident, incorporating this information into the individuals record.
Social Worker Qualifications:
- Minimum of Bachelors Degree from an approved school of Social Work and a Bachelors Degree in Gerontology.
- Experience in a Skilled Nursing Facility preferred.
- Possess the ability to deal tactfully with personnel, residents, family members, visitors and the general public.
- Ability to deal with or counsel with hostile and emotionally upset residents and family members.
