The Special Programs Coordinator works as part of the care team and assists 1st Choice Healthcare Providers in providing population health care. This position is responsible for the implementation and ongoing management of clinical and operational programs related to value-based care. These programs include Patient Centered Medical Home (PCMH) and participation with a designated Accountable Care Organization (ACO). The Special Programs Coordinator is directly responsible for administration of the PCMH program, including annual reporting and recertification of all recognized sites; acts as the designated liaison and program manager for the ACO; directly supervises the nursing staff responsible for value-based care programs including Transitional Care Management (TCM), Chronic Care Management (CCM), and Annual Wellness Visits (AWV).

This is a Safety Sensitive position(the use of a Medical Marijuana card is prohibited according to Act 593.

Qualifications

  1. Minimum - 3-5 years in healthcare with supervisory or administrative experience or associate's degree in a healthcare-related field.
  2. Working knowledge value-based care initiatives and population health programs preferred.
  3. Excellent communication and strong interpersonal skills.
  4. Ability to work effectively with people of all backgrounds.
  5. The ability to work cooperatively with professionals and other staff, patients, and visitors.
  6. A conscientious individual with the ability and willingness to accept responsibility.
  7. Able to work in a demanding setting.
  8. Some traveling between clinics is required.
