As a Physical Therapist, you will play a vital role in helping older individuals maintain their independence and quality of life. You will work with an interdisciplinary care team to provide personalized care and rehabilitation services to the frail, elderly, and long-term care population.


What youll be doing:

  • Provide physical therapy services to participants
  • Develop and implement individualized care plans in collaboration with the interdisciplinary care team
  • Conduct assessments to evaluate participants' physical abilities and limitations
  • Design and deliver therapeutic exercises and interventions to improve mobility, strength, and function
  • Monitor progress and adjust care plans as necessary
  • Educate participants and their families on techniques to enhance mobility and prevent injury
  • Maintain accurate and timely documentation of services provided


Requirements:

  • Current, unencumbered, active license to practice as a Physical Therapist in the State of Michigan
  • Experience working with the frail, elderly, or long-term care population
  • Strong assessment and therapeutic skills
  • Ability to work collaboratively with an interdisciplinary care team
  • Excellent communication and interpersonal skills
  • Compassionate and patient-centered approach to care
