The Position:

The Permanent Housing Placement Liaison is responsible for coordinating all aspects of the move-in process for formerly homeless individuals and families, to assist with easing the transition from homelessness to being permanently housed. This position will be responsible for ensuring smooth coordination of services between departments. The Permanent Housing Placement Liaison will interact with a diverse staff, community partners, and furniture vendors and must be able to coordinate move-in activities (warm handoffs, furniture orders, lease collection) quickly and smoothly upon short notice to a seamless transition for participants.

What You’ll Do:

  • Oversees participant transfers and ensures that all transfers are made to the appropriate department and processed in a timely manner.
  • Ensures consistent communication with internal and external stakeholders providing appropriate updates and warm handoffs.
  • Supports weekly meetings with department managers to review participants transitioning into housing.
  • Ensures all move-in protocols are being followed including collecting and auditing leases, and ensuring resolution of all pending move-ins.
  • Supports accurate housing transition tracking mechanisms and conducts weekly audits to ensure accuracy of records.
  • Responsible for supporting the reviewing, submitting, and coordination of furniture orders and keeping within the allowed amounts or requesting appropriate adjustments as needed; submits purchase orders.
  • Support the complete transfer process, including tracking housing transitions and managing associated pending items.
  • Accurately enter Move-In dates into HMIS Clarity.
  • Coordinate, attend, and oversee scheduled lease signings with all involved case workers.
  • Deliver security deposit/rental assistance checks at lease signings.
  • Obtain and process lease corrections and revisions as needed.
  • Assists housing staff with identifying resources such as donations and cell phones that will aid with the participants' housing success.
  • Attends required trainings; seeks learning opportunities for continued education; stays up to date on the best practices in ending homelessness and participant care.
  • Attends internal and external meetings as required/requested by supervisor.
  • Drives personal vehicles in and around Los Angeles County and drives agency vehicles periodically to oversee lease signings.
  • Ability to work a flexible work schedule, including evenings and weekends as needed.
  • Additional tasks, projects, and responsibilities as assigned by supervisor.

What You’re Skilled At:

  • Knowledgeable of tenant rights, standard lease agreement adherence, and community resources.
  • Basic knowledge of tenant/landlord laws, specifically on lease accuracy.
  • Strong interpersonal skills, cultural competence, and ability to employ sensitivity in building rapport with clientele; ability to network and build relationships quickly and easily.
  • Practical knowledge and use of HMIS or other electronic database systems.
  • Experience utilizing Excel to capture and synthesize data.
  • Strong advocacy skills and ability to act as a liaison between various parties to meet needs.
  • Creative and solution-focused problem-solving skills.
  • Must be able to perform extensive charting, electronic data entry, and documentation.
  • Excellent written and verbal communication skills.
  • Ability to work in a team and independently.
  • Bilingual: Spanish/English preferred, but not required.
  • Knowledge of Housing First Model, Harm Reduction, Motivational Interviewing, and other best practices serving formerly homeless individuals.

OTHER:

  • Maintain and execute confidential information according to HIPAA standards.
  • Obtain and maintain CPR/First Aid Certification.
  • Ability to pass post-offer Tuberculosis (TB) clearances.
  • Travel is a regular duty for this position and is required 75% of the time.
  • Use of a personal vehicle to travel between worksites and other locations is required.
  • Must have and maintain a valid California Driver's License and auto insurance in good standing.
  • Ability to work a 9/80 work schedule.
