JOB DESCRIPTION
The Intake Coordinator/Marketer is expected to perform a variety of duties in the area of clinical planning. The Intake Coordinator is expected to oversee all intake responsibilities and ensure the highest quality assurance in order to provide the highest quality service to clients.

ESSENTIAL JOB FUNCTION/RESPONSIBILITIES
- Oversee all intake
- Evaluating patients referred to home care
- Planning, writing, and coordinating referrals for home care services
- Collaborating with the hospital's case managers
- Discharge planning
- Utilization review
- Ensure case management to facilitate safe discharge
- Completing intake referral information including intake data, essential background information, insurance verification, hospital course, and plan of care
- Making marketing calls to referral sources
- Coordinate referrals with the Area on Aging
- Other duties as assigned
POSITION QUALIFICATIONS
- Previous intake experience in the home care field is required.
- Two years of management experience in home health or related home health care organization preferred.
- Demonstrated ability to direct professional administrative personnel.
- Ability to meet and deal tactfully with customers/patients and the community.
- Has excellent observation, verbal, and written skills.
