This role supports the Transition Coordinator in managing and implementing transition plans and activities. The Transition Assistant is responsible for providing students with the support and resources necessary for a successful transition to post-secondary education and/or the workforce and for performing varied duties to support the site.


EDUCATION AND EXPERIENCE

  • Bachelor's Degree required in Psychology, Guidance Counseling, Social Work, Special Education, or a related field.
  • Master's Degree preferred
  • At least 2 years of experience working with high school students (preferably those with Intellectual disabilities and/or Autism)
  • Prior college counseling and/or vocational coaching experience is strongly preferred


KNOWLEDGE, SKILLS & ABILITIES

  • Ability to communicate effectively with a variety of audiences, orally and in writing, including electronic media
  • Excellent time management skills and ability to organize, prioritize, and manage work assignments in an efficient manner and within established timeframes
  • Operating knowledge of computer programs, including email, word processing, spreadsheets, databases, and web-based technologies and platforms
  • Knowledge of record-keeping procedures
  • Ability to collect, summarize, analyze, and graph data
  • Ability to establish and maintain collaborative working relationships with all stakeholders
  • Ability to work collaboratively with others and facilitate groups to consensus
  • Be able to relay information regarding the fundamentals of ACCES-VR and other adult services funded through NYS including but not limited to OPWDD and OMH
  • Travel is required throughout Long Island (a Valid Driver’s License is a must)


ESSENTIAL DUTIES & RESPONSIBILITIES

The following statements are intended to describe the general nature and level of work being performed by individuals assigned to this position.

 

Supervisory Capacity Role:

  • Supervise staff in the field enabling students/clients to be able to perform effectively in community-based worksites using instructional strategies.
  • Attendance at professional development training regarding regulatory issues and changes in processes related to the job. Ensure all student scheduling reports, planners, and district activities are submitted on time and accurately.
  • Attendance at all regional meetings or meetings requested through company admin in regard to the implementation and operations of services is required.
  • Monitor students at employment sites, job training sites, and/or community-based instructional sites with a Career Consultant.
  • Develop a thorough knowledge of employment opportunities and/or training programs in which students will be placed, and perform all tasks prior to and concurrent with placement, as needed.
  • Acquire background information, including current known academic levels and limitations of students, and use this information to identify potential job placements. Match students with employees and post-secondary opportunities for optimum success and meet with employers/employees at sites prior to student placement by conducting a Job Analysis Survey.
  • Monitor the collection and record of information on student performance through progress monitoring using Task Analysis and Industry Standard data.
  • Ensure that field staff enter all appropriate inventories, assessments, and reports pertaining to their student’s transition or ACCES client into company systems.
  • Assist Career Consultants in student behavior using positive behavior supports and behavioral management procedures.
  • Supports field staff Consultant in clerical tasks associated with instructional activities, preparation of classroom activities, and preparing/collecting materials and equipment as needed.
  • May attend Committee of Special Education (CSE) meetings, as requested

Perform other duties as assigned


Coverage Capacity Role:

  • Job coach students at employment sites, job training sites, and/ or community-based instructional sites in the event a Career Consultant is not present.
  • Collect and record information on student performance through progress monitoring using Task Analysis and Industry Standard data.
  • Assist in student behavior using positive behavior supports and behavioral management procedures with district staff members.
  • Perform clerical tasks associated with instructional activities, prepare the classroom for activities, and prepare/collect materials and equipment as needed.
  • Provides individual and group work-readiness skills training to students in districts and through ACCES-VR in areas including resume development, interview skills, networking, employer expectations, independent living skills, positive work attitudes, developing initiative, motivation, and responsibility under the direction

of certificated staff.

  • Perform and enter all appropriate inventories, assessments, and reports pertaining to the student’s transition or ACCES VR client into company systems.
  • Provides training and job coaching as needed in community-based work environments by assisting

employers with the orientation of students to work methods, specific assignments, work rules,

and providing scaffolding in preparation for fading supports.

  • Works with students and employers to identify student strengths and needs as related to vocational

skills, job accommodations, and solutions to employment barriers.

  • Initiates discussions with students regarding post-secondary careers, educational, and independent

living options and goals, and may assist with linkages to community agencies and resources.

  • May attend Committee of Special Education (CSE) meetings, as requested.

Perform other duties as assigned


JOB DEVELOPMENT

  • Provide necessary job development for placement into internships, job placement, job exploration, and/or mentor programs for the clients we serve.
  • Work with supervisor to develop marketing plans for student/consumer needs and district programs requirements.
  • Contribute routinely to the job bank of businesses that are interested in participating with CEO in some capacity.
  • Utilization of the CEO strategies for job development.
