We are hiring a Certified Peer Support- Case Manager to join our Assisted Outpatient Treatment (AOT) Program, located in Seattle. As Certified Peer Support, you'll provide vital support and resources to peers enrolled in the AOT program, while utilizing your own life experiences to encourage and guide individuals on their pathways to recovery. From offering peer recovery services to facilitating outreach and engagement efforts, you'll play a crucial role in promoting self-advocacy, community living skills, and overall well-being. The pay for this role is $76,600 to $81,300, annually, DOE.

Requirements:
  • Provides all appropriate and approved peer recovery support services to patients referred or enrolled in the AOT program.
  • Provide outreach and engagement services to complete initial engagement and maintain retention in treatment services, especially for patients who have difficulty engaging in treatment.
  • Assist patients with obtaining needed resources and support through care coordination and case management services through advocacy. Examples of resources and support including housing, daily living activities, transportation, basic needs, financial planning, vocational needs, additional behavioral health needs, etc.
  • As a role model for recovery, assist patients in developing and moving forward with their recovery goals. Document peer-related goals, objectives, and interventions in patient’s EMR treatment plan in a timely and complete manner.
  • Provide interventions and de-escalation assistance, when necessary.
  • Provide a positive, empathetic, and non-judgmental treatment setting to all patients.
  • Utilizes agency-provided vehicle to provide outreach services to AOT patients and transport patients to and from approved resources and services within the community.
  • Always maintains patient confidentiality.
  • Documents all services and interventions provided into individual patient electronic medical record (EMR) in a timely manner.
  • Submit any required documentation and paperwork to supervisor and/or manager in a timely manner, per AOT contract requirements.
  • Attends ITA and AOT court hearings, as assigned.
  • Provide care coordination efforts with internal and external providers to assist patients in achieving their treatment plan goals and objectives.
  • Other duties as assigned.


Minimum Qualifications:

  • Minimum high school diploma or GED and continuing education in SUD related field.
  • WA State Peer Counselor Certification required.
  • WA State Agency Affiliated Counselor credentials required.
  • WA State Driver’s License and a clean driving record (driving record background check will be obtained).
  • At least three years’ experience as a WA State certified Peer counselor.
  • Extensive knowledge of community resources and ability to facilitate/coordinate access to those resources.
  • Experience in working with individuals who are diagnosed with a substance use disorder and/or mental health.
  • Ability to work with and maintain positive relationships with a diverse group of participants, co-workers, and community service providers.
  • Ability to learn and adapt to new electronic record systems relevant to the organization's operations, including Microsoft Outlook and Word.
  • Demonstrated ability to adapt to changing environments and manage multiple priorities effectively
