The opportunity
We are seeking a full-time Virtual Nurse Practitioner to join our One Medical at Home, high-risk care team and our High Risk Care Management virtual team, supporting our patients in multiple markets (Atlanta, Seattle, Phoenix, Denver, Houston) and other markets as needed. The Virtual Nurse Practitioner will care for geriatric, frail, and clinically complex patients through a team partnership with the High Risk Care Medical Director and high risk teams, as well as the in office Primary Care Providers.

Once through orientation, independently manage patients with multiple, complex conditions through a balance of tele-assist home visits (video visits with field care coordinators in the home) and telephone/video interventions.
- Complete thorough initial visits with patients, diving deep into medical, social and behavioral context.
- Complete ongoing follow up visits with patients, focusing on geriatrics and chronic condition management.
- Complete post-discharge visits on recently admitted patients, including comprehensive medication reconciliation, Specialist coordination, and chronic condition management
- Complete engagement and outreach visits on patients who have not been seen by their primary care provider this year or who have chronic condition opportunities
- Work directly with the patient and through the multidisciplinary care team to impact ongoing care with particular emphasis on preventable hospitalizations and ensuring care aligned with patient goals.
Collaborate and coordinate with the patient's primary high risk care team and primary care team and specialists. Participate actively in team huddles to review care plans and assess patient goals of care and care plans.
With support of the team, respond to patient and patient's family telephone calls.
Participate in the market after-hours call pool.
What you’ll need
- 5+ years of home based, in office or virtual complex care, Geriatric Primary Care, geriatrics or palliative care experience as a Nurse Practitioner
- Currently licensed in Georgia and at least one additional state (Arizona, Colorado, Texas, or Washington)and able/willing to obtain licensure in the states where the provider does not hold a license.
- Residency in Georgia is required for role
- Flexibility to work in Eastern, Central and Mountain Time Zones, as assigned
- Spanish Speaking preferred
- Completed an accredited NP program with a national certification (ANCC, AANP) in Family, Geriatrics, or Adult
- Experience with home-based care preferred
- Experience with value-based care principles and risk adjustment
- The ability to build successful relationships with team members and communicate effectively both 1-on-1 and in groups
- The ability to work with care coordinators to direct care in the home through tele-assist capabilities
- Strong technology skills and ability to use telehealth platforms
