We are currently looking for a Physical Therapist to join our team. The position would be responsible for  providing physical therapy evaluation and treatment to patients in their homes or alternate community settings.



Responsibilities:
  • Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
  • Assesses and evaluates client for physical therapy needs and develops and implements appropriate plan of action in collaboration with the physician
  • Documents all changes in client’s condition and updates care plans and outcomes accordingly
  • Identifies counseling needs and plans appropriate teaching strategies
  • Monitors quality and appropriateness of services provided
  • Adheres to agency infection control and safety policies
  • Adheres to the organization’s policy in regards to absenteeism and appearance
  • Omission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this position

Qualifications:
Physical Therapy Licensure in NJ required2-3 years of prior clinical experience and community based experience preferredStrong computer skillsStrong communication and interpersonal skillsValid NJ drivers license 

Working Conditions/Physical Demand: Working indoors/outdoors, driving, lifting, sitting, standing, walking, fine motor hand movement, use of phone and tablet for documentation
