As the Rehabilitation Coordinator you will collaborate with the health care team and referral sources to manage referrals, identify appropriate admissions, and ensure an effective transition of care to The Center for Rehabilitation.

Principal Duties and Responsibilities:

  • Coordinate the pre-admission process
  • Document appropriately in electronic medical record and admissions-supporting platforms
  • Provide summaries of admission and pre-admission documentation, reviewing referrals with the Medical Director
  • Collaborate to obtain pre-certification from third party payors prior to the patient's admission. Obtain precertification independently when necessary.
  • Provide educational materials to patients and their families. Clarify and set the expectation of care to foster optimal clinical outcomes and support a great patient experience.
  • Educate referral sources regarding the admissions process and assist them in identifying candidates for admission
  • Communicate effectively via telephone and in-person with patients, physicians, third-party payers, employers, providers, vendors, and other healthcare members
  • Act as an expert consultant to members of the health care team. Monitor the on-going process of gathering information about the patient referral to support expedient determination of referral.
  • Build relationships with attending physicians, the healthcare team, patients and family. Collaborate with ancillary personnel on formulating an admissions plan.
  • Attend meetings and huddles, offering feedback to support process and teamwork
  • Assist with data collection and implementation of action plans for CMS, JCAHO, AHA Quality Initiatives as well as ChristianaCare defined goals and measures
  • Work collaboratively with hospital PI efforts
  • This position will require travel between ChristianaCare and referral locations

Education and Experience Requirements:

  • BS or higher level degree in Nursing; or Master’s degree in Social Work (MSW); or BS or higher degree in Physical, Occupational, or Speech Language Therapy
  • At least three (3) years clinical rehabilitation experience and/or insurance rehabilitation experience
  • Active Delaware license as an RN, Social Worker, Physical Therapist, Occupational Therapist, or Speech Language Pathologist required
  • Related work experience in Utilization Management, Case Management, and/or Home Care preferred
