Patient Care Manager Position in Columbia, SC
Overview:
We are seeking a dedicated and compassionate Healthcare Coordinator to join our team in Columbia, SC. As the Patient Care Manager, you will be responsible for providing direct management and leadership to our clinical support staff and field clinicians, ensuring high-quality care for our patients.

Job Description:
As a Patient Care Manager, you will:
Communicate and coordinate care needs reported by case managers.
Ensure updated schedules, continuity of care, and delivery of quality care by the clinical team.
Facilitate case conferences and follow-up actions related to meetings.
Coach and mentor clinicians and clinical support staff, addressing development needs and achieving KPIs.
Handle voluntary attrition of assigned team and provide support and solutions for personnel concerns.
Supervise and direct team members, assign visits, and ensure timely completion of tasks.
Manage the site in the absence of higher management.
Oversee clinical, administrative, and recordkeeping functions.
Respond to phone inquiries in a timely and professional manner.
Participate in on-call nursing schedule and provide direct skilled bedside nursing care in patients' homes.

Requirements:
Registered nurse with current licensure to practice nursing in South Carolina.
CPR certification.
Previous experience in a home care setting with at least two years of management or supervisory experience (preferred).
Ability to work effectively in an interdisciplinary setting.
Valid driver's license and insured automobile in good working order.
Knowledge of professional standards, Medicare guidelines, and regulatory requirements.
Excellent observation, judgment, and communication skills.
