As a Home Health Licensed Practical Nurse (LPN) with Faith, you will deliver personalized skilled nursing care to patients based on a comprehensive, patient-specific care plan established by the RN Case Manager. This plan will set goals based on nursing diagnoses and include preventative and rehabilitative nursing actions, ensuring you have the support and collaboration of a full team of healthcare professionals.

Key Responsibilities:
As a Home Health Licensed Practical Nurse (LPN), your responsibilities will include but are not limited to:

  • Delivering skilled nursing care in the home as outlined in the treatment plan and nursing care plan.
  • Assessing patients' conditions, effectively communicating, and accurately documenting any relevant information regarding their health.
  • Promoting education for patients, families, and caregivers, taking into account the needs of the patient and the standards of care.
  • Performing wound care, managing diseases, and reviewing medications in the home setting as directed by the physician and supervising RN.
  • Collaborating as a team member with the patient, physician, and interdisciplinary team.

LPN Qualifications:

  • Must possess a current, valid, unencumbered Missouri & Kansas State Board of Nursing LPN license (compact or multi-state license preferred).
  • Minimum of 1 year of home health experience required.
  • Valid driver's license, reliable transportation, and auto insurance are necessary.
  • Current CPR certification is required or must be obtained.
