How You Will Make a Difference

As we expand our services in new and existing markets, we're looking for a Patient Services Coordinator who shares our values of being Trustworthy, Capable, Compassionate, Proactive, and most importantly, called to serve patients in their homes. The Patient Services Coordinator serves as a bridge linking patients to the care they need. In this role, you work behind the scenes orchestrating the details between the patient, family, care team and other care providers to ensure timely care is provided and patient needs are fully addressed.

As the Patient Services Coordinator, you will:

  • Schedule the patient services and coordinate with the patient, family and care team members.
  • Ensure that all agency field personnel are properly scheduled to ensure expected levels of productivity are achieved and care is provided in the most efficient manner possible.
  • Ensure field caregivers have the information they need to appropriately care for the patient.
  • Provided care to patients as needed

  • Familiar and comfortable with technology. HCHB experience is a plus

Experience to Deliver on our Mission:

  • High school diploma, valid state driver’s license and auto liability insurance
  • Current Vocational/Practical Nurse License preferred
  • One year experience in home health or hospice preferred
