The Lead, Primary Care Physician (Lead, PCP) is the initial step in the clinical leadership track at ChenMed; the Lead PCP will demonstrate:
- Accountability for outcomes: The Lead PCP demonstrates accountability for outcomes, strong clinical care, and cost-effectiveness for each patient in their panel of at least 360 patients.
- Coaching for health: The Lead PCP acts as a health coach, helping patients set health goals and frequently follows up on those goals.
- Simplifying for action: The Lead PCP simplifies and prioritizes appropriately to make behavior change actionable for patients and care teams.
- Leadership: Leads Super Huddle (SH) and Transforming Care Meeting (TCM) weekly and supports Center Manager/Center General Manager as needed.
We are an outcomes-focused, value-based organization and for their panel of patients, the following metrics are regularly measured to help Lead PCPs become successful: patient admissions/thousand, appointment slots per day, HEDIS>4, CG CAHPS, clinical gaps closures, and medical cost effectiveness relative to center target. Each Lead PCP will have goals for these metrics and will be expected to work towards those targets with their center and market leadership.
ESSENTIAL JOB DUTIES/RESPONSIBILITIES:- Independently provides care for patients with acute and chronic illnesses.
- Takes full accountability for patient care and outcomes, seeking consultation from specialists when needed.
- Engages with hospitalists for hospitalized patients.
- Responsible for assessment, diagnosis, treatment, management, education, health promotion, and care coordination for patients.
- Leads their care team consisting of care promoter, care facilitator, and care coordinator.
- Engages with transitional care team for patients unable to come to the office.
- Plays an active role in the management of their center and helps cover for other providers.
- Performs other duties as assigned.
Available and Accessible for patients to build trust. It is expected that the Lead PCP will be open and available for walk-in visits and respond to messages in a timely manner.
EDUCATION AND EXPERIENCE CRITERIA:- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty required.
- Must have a current, active State Medical License.
- Board certification in Internal Medicine, Family Medicine, Geriatrics, or similar specialty preferred.
- Must have a current DEA number for controlled substances.
- Basic Life Support (BLS) certification required within the first 90 days of employment.
