Step into a Leadership Role in Patient-Centered Care

At Elara Caring, we believe in delivering care where it counts—compassionately, professionally, and precisely when and where it’s needed most. As a Licensed Practical Nurse Clinical Coordinator (LPN CC) in our Hospice team, you will be instrumental in ensuring seamless patient care by supporting Clinical Team Managers (CTMs) and coordinating the care journey for our patients. In this hybrid role, you’ll balance time in the office with field support, making a real difference in patient care, communication, and team efficiency.

Why Elara Caring?

  • Work-Life Balance & Flexibility: Enjoy a balanced Monday-to-Friday schedule with autonomy.
  • Meaningful Impact: Provide 1:1 patient care where your efforts directly influence patient comfort and quality of life.
  • Supportive & Collaborative Culture: Work in an environment that values your contributions and encourages growth.
  • Competitive Compensation & Benefits: Receive competitive pay along with a comprehensive benefits package.
  • Career Development: Take advantage of tuition reimbursement and ongoing educational opportunities.
  • Comprehensive Health Coverage: Access to medical, dental, and vision insurance, with $1 copays for Tele-Health visits.
  • 401(K) with Immediate Employer Match: Plan for your future with vested matching contributions from day one.
  • Paid Time Off & Unique Benefits: Paid holidays, family and pet bereavement leave, plus pet insurance.

Your Responsibilities As An LPN Clinical Coordinator

In this role, you’ll serve as the backbone of patient care coordination, facilitating smooth communication and optimal care outcomes for patients and their families. Key responsibilities include:

  • Care Coordination: Maintain accuracy in documentation through Homecare Homebase (HCHB) to ensure all workflow standards are met.
  • Communication Bridge: Serve as the main connection between patients, CTMs, medical staff, and facilities, enhancing communication and care coordination.
  • Patient Advocacy: Assess and respond to patient needs, advocating on their behalf to ensure the highest quality of care and compassion.
  • Decision-Making & Problem Solving: Use sound judgment to make decisions, manage complex situations, and escalate issues when necessary.
  • Conflict Resolution: Address and resolve complaints effectively to ensure positive outcomes for patients and staff.
  • Leadership & Team Support: Foster a collaborative work environment, offering guidance and support to help your team thrive.

What You’ll Need To Succeed

  • Licensed Practical Nurse (LPN): Graduate of an accredited nursing school with an active LPN certification.
  • Relevant Experience: 1+ year of practical experience, ideally in Home Health or Hospice care.
  • Strong Communication Skills: Ability to communicate clearly, actively listen, and adapt messaging as needed.
  • Time Management Skills: Organized, efficient, and able to handle multiple priorities.
  • Tech-Savvy: Proficient with systems like Homecare Homebase, Workday, and Microsoft Office.
  • Patient-Centered Focus: Skilled in understanding and addressing patient needs, with a commitment to advocating on their behalf.
  • Bilingual (Preferred): Proficiency in Spanish is a plus.
  • Valid Driver’s License & Reliable Transportation: Required for travel in support of field duties.
