Job Summary:

The Social Services Director will oversee and coordinate all aspects of social services within our facility. This role is vital in ensuring that residents receive comprehensive support tailored to their individual needs. The ideal candidate will have a strong background in social services management, a deep understanding of regulatory requirements, and a passion for improving the lives of seniors.

Key Responsibilities:
  • Develop, implement, and oversee social services programs and initiatives that meet the needs of our residents.
  • Conduct assessments to identify residents' psychosocial needs and develop individualized care plans.
  • Collaborate with interdisciplinary teams to ensure the effective delivery of services and support.
  • Maintain compliance with state and federal regulations, including documentation and reporting requirements.
  • Provide leadership and supervision to social services staff, including training, performance evaluations, and professional development.
  • Act as a liaison between residents, families, and community resources to address concerns and facilitate communication.
  • Coordinate and lead resident and family meetings to discuss care plans, progress, and any adjustments needed.
  • Develop and implement community outreach programs and partnerships to enhance services and support for residents.
  • Monitor and evaluate the effectiveness of social services programs, making adjustments as necessary to improve outcomes.

Qualifications:
  • Bachelor's degree in Social Work, Psychology, Human Services, or a related field (Master's degree preferred).
  • Valid Social Work License (LCSW or equivalent) is preferred.
  • Minimum of 5 years of experience in social services, with at least 2 years in a supervisory or management role within a healthcare or senior care setting.
  • Strong knowledge of state and federal regulations related to social services in senior care.
  • Excellent interpersonal and communication skills, with the ability to work effectively with residents, families, and staff.
  • Demonstrated leadership abilities, with a commitment to fostering a supportive and collaborative team environment.
  • Strong organizational and problem-solving skills, with the ability to manage multiple tasks and priorities.
