Case Manager RN

Type: 4 month Contract, with potential for extension
Schedule: 7:30-4 pm , Mon - Fri
Pay: $45 to 47/hr
Locations: Occasional travel between Everett and Silver Lake offices
Responsibilities of the Case Manager RN:
• Hospital Transition calls for Medicare Advantage patients
• Care Coordination for complex, high-risk patients
• Motivational interviewing with patients and families
• Palliative Care conversations when appropriate
• Participation in Interdisciplinary Team meetings
Qualifications for the Case Manager RN:
• Active and unrestricted RN license in WA
• Associate degree or higher in Nursing (RN)
• Ambulatory Care
• Outpatient Care Coordination
• Difficult conversations
• Care Management
• Hospital Transition work
• Immunizations required: MMR, seasonal flu shot, Varicella, TB Blood Test, Hep B., Tdap

Pay Details: $45.00 to $47.00 per hour
