Schedule/Day in the Life

The RN Case Manager works one day a week in person at the Clinic location and the other four days remote (M-F 7:30am-4pm). They must choose one day a week to come in consistently (e.g. the RN decides with their manager to come in every Thursday).


Overview

The RN Case Manager is a part of the post hospitalization team at the Clinic and is assigned to patients. This is a contract position until September, but if a candidates excels in the position, they may have the opportunity to be considered for full time openings if available.


Patient Population

The RN Case Manager has a caseload of around 30 elderly patients. The patient panel is made up of patients that were hospitalized short term and needing temporary case management services, patients hospitalized long term and require long term post discharge support, and patients that were referred from providers within the Clinic.


Start Date: ASAP


Pay: $40-$50 an hour (depending on experience)

Requirements:

  • RN License in WA State
  • Prior case management experience in an outpatient setting (preferably internal medicine) is ideal. Candidates with home health or hospice case management experience will also be considered.
  • Experience with hospital transitions is preferred


Soft Skills:

  • Passion for working with elderly patients
  • Nurse autonomy to make decisions on patients care
  • Strong communication skills for working with patients and families.
  • Being able to provide patient education and "motivational interviewing" (motivating the patients to change their behavioral and lifestyle to meet their healthcare goals)
  • Confidence and ability for public speaking to present patient cases to the care team
