Your New Role:

This position provides engagement and clinical case management services with an interdisciplinary, multi-agency team to high-risk youth (ages 9-18) who are homeless or at-risk of homelessness. The person in this position will carry a caseload of youth that speak Spanish primarily and will routinely be required to converse in Spanish with youth and youth family members. The responsibilities of this position include intake and assessment; housing placement and stabilization; individual mentoring and case management; crisis intervention; case planning and contracting; resource brokerage and referral; coordination and advocacy; follow-up and after care; provision of clinical and financial and other record keeping related to job duties.

What this Role is...

Creative- Provides dynamic support to youth and families, following up and following through to meet youth where they are at, advocating and collaborating to meet individual self-identified needs, navigating resources and systems.

Relationship-Driven- Statistics show one of the most significant protective factors for youth is being able to identify just one safe adult in their life. In this role, you will build individual relationships with a caseload of 15+ young people through compassionate boundaries, transparent communication, and goal-oriented advocacy.

Community-Based - In this role you will be working with youth primarily in community and providing one-on-one in person service. This can entail meeting youth at their homes or at school, driving youth in your car, and accompanying youth to things like doctors' appointments, and more.

Skills & Attributes:

* Fully fluent in speaking both English and Spanish

Desire to work with multicultural youth and staff in a diverse setting.

* Proven ability and commitment to working with BIPOC and LGBTQIA2S+ youth, staff, and community members as well as other providers, particularly with understanding of Trauma-Informed Care, Positive Youth Development and Strength-Based approaches.

* Strong interpersonal and communication skills, including detail-oriented coordination and documentation.

* Must have access to an automobile, a valid driver's license, and active auto insurance on that vehicle.

* Must be willing to be CPR trained and certified.

Experience:

* A minimum two years of experience working with high-risk youth and families

* Prior experience providing crisis intervention and/or case management

* Have significant work and life experience OR a college degree in social work, psychology or related human services field
