Summary

Maine Medical Partners' (MMP) has an exciting position for an RN Diabetes Educator to move into a Clinical Manager leadership role. For this new position, we are looking for a Registered Nurse who is an innovative thinker and is passionate about diabetes education. This role provides you with the opportunity to:

  • Take diabetes education to higher levels to meet patient needs in an ever-changing environment.
  • Bring creative ideas to this MMP Practice to build new & enhance existing programs.
  • Collaborate with other MaineHealth Diabetes Educators to help build strong education programs system wide.
  • Oversee a team of Diabetes Educator professionals by providing ongoing leadership and mentoring.
  • Provide patient care through patient triage and education.

An RN license and having your Certified Diabetes Educator Certification (CDCES), or exam ready, are required. Insulin Pump certification must be up-to-date or be in the process of re-certifying. Prior experience working in the Diabetes field is a must for success.

This is a full-time, Monday-Friday, day-shift position. Once fully trained, there is the opportunity to work a hybrid schedule of 2 days at home depending upon the needs of the practice.

  • Position Summary
    • In addition to direct patient care the Diabetes Education Program Manager functions in a leadership role, coordinating Diabetes Educator scheduling and staffing at the primary clinic site as well as outreach clinics, and acts as a content expert resource. The Diabetes Education Program Manager insures the education program is in compliance with National Standards for Diabetes Self-Management Education, including insurance carrier programs guidelines and requirements pertaining to reimbursement. The Diabetes Education Program Manager serves as a liaison with the inpatient team collaborating on patient care improvement projects, and facilitates continuity and consistency of care.
  • Required Minimum Knowledge, Skills, and Abilities (KSAs)
    • Education: BSN preferred
    • License/Certifications: Active, unencumbered, State of Maine Nursing License. Certified Diabetes Educator certification or expectation to sit for exam within 6 months. Current insulin pump certifications
    • Experience: Five years of experience in the clinical setting, and is familiar with the lifelong process of managing a chronic disease. Knowledge of Risk Management preferred
    • Excellent communication, interpersonal and judgment skills.
    • Ability to interpret the medical record and interpret quality and incident data
    • Ability to rapidly triage patient needs, prioritize and delegate work in a rapidly changing environment
    • Interpersonal and communication skills necessary to interact effectively with patients, families, and other health care providers.
    • Analytical ability sufficient to identify patients at risk and initiate appropriate action.
    • Ability to identify potential crisis situations involving patients and/or families, and to plan strategies for managing these situations when they arise.
    • Analytical ability required gathering and interpreting information necessary to resolve problems in an accurate and timely manner. Ability to concentrate and pay close attention to detail.
    • Ability to maintain confidentiality
