Job Summary: The Care Coordinator works to improve patient outcomes by coordinating and facilitating patient care across a multidisciplinary care continuum while ensuring optimum utilization or resources and compliance with associated requirements. Coordinates assessment, planning, and implementation of health care and related services to assigned patients. Patient age groups range from infant to old age or death.
Minimum Requirements: Associate's degree or Bachelor's degree in nursing, or Bachelor's degree in Human Services/Social Services field with three to five years relevant hospital social service work experience. If nursing degree, must have North Carolina license. Three to five years experience in acute health care setting preferred.
