CHI Health has an immediate need to add an amazing Wellness Clinic RN (Nurse) to our Midlands Family Medicine Clinic located off of Hwy 370 & 84th St

Hours: Monday-Friday 8a-5p, NO weekends & NO holidays

**Responsibilities**

****$5000 Sign On Bonus Offered****

This position will support our healthcare teams by ensuring they have everything they need in order to care for our patients. They also play a huge role in a patient's experience and prep when they visit any of our clinics. **Expectations:**

+ This position is for a Annual Wellness Visit RN. Wellness RN Work as part of a team to provide Annual Wellness visits to current Medicare patients past their 1st year of eligibility.

+ Help the patient and provider to create custom 5-10 year preventative screening plans for Medicare patients.

+ Provide education to help the patients understand how preventative care impacts their health.

+ Performs general nursing care to patients by following established clinic workflows.

+ May perform specific nursing care as it relates to specialty of the practice.

+ Educate patients and families about health status, health maintenance, and management of acute and chronic conditions.

+ Document patient assessment and intervention data using established medical record forms/automated systems and documentation practices.

+ Performs other duties within scope of role as assigned.

**Qualifications**

**Minimum Qualifications:**

+ Current Registered Nurse (RN) license that allows you to practice in our state

+ Graduate of an accredited nursing program

+ Basic Life Support (BLS) from the American Heart Association required

**Pay Range**

$26.38 - $38.26 /hour
