CHI Health has an immediate need to add an amazing Wellness Clinic RN (Nurse) to our Midlands Family Medicine Clinic located off of Hwy 370 & 84th St Hours: Monday-Friday 8a-5p, NO weekends & NO holidays CHI Health strives to care for you the way you care for your patients. This position will support our healthcare teams by ensuring they have everything they need in order to care for our patients. They also play a huge role in a patient’s experience and prep when they visit any of our clinics. We couldn’t do it without you If you have a passion for supporting others and enhancing patient experience, we want to talk to you. Expectations: This position is for a Annual Wellness Visit RN. Wellness RN Work as part of a team to provide Annual Wellness visits to current Medicare patients past their 1st year of eligibility. Help the patient and provider to create custom 5-10 year preventative screening plans for Medicare patients. Provide education to help the patients understand how preventative care impacts their health. Performs general nursing care to patients by following established clinic workflows. May perform specific nursing care as it relates to specialty of the practice. Educate patients and families about health status, health maintenance, and management of acute and chronic conditions. Document patient assessment and intervention data using established medical record forms/automated systems and documentation practices. Performs other duties within scope of role as assigned. Qualifications Minimum Qualifications: Current Registered Nurse (RN) license that allows you to practice in our state Graduate of an accredited nursing program Basic Life Support (BLS) from the American Heart Association required Pay Range $26.38 - $38.26 /hour We are an equal opportunity/affirmative action employer.
