COMPLEX CARE NURSE COORDINATOR

* Acts as a case manager expert and mentor to identify and resolve issues impacting quality, safety and timeliness of care transitions and escalates to appropriate stakeholders as appropriate
* Co-manages with unit-based case managers complex patient care discharges and collaborates with relevant clinical teams both internally and externally.
* Identifies potential discharge barrier mitigation strategies to ensure care progression and safe care transitions
* Identifies new escalation pathways and shares with case management team, providing valuable resources and updates
* Responsible for executing and enforcing best-practices, and new practices (e.g., alternate care pathways) related to discharge planning.
* Provides mentorship and guidance to clinical teams and case management as needed related to complex care and post-acute discharge
* Monitors ongoing performance of initiatives and develops recommendations/identifies opportunities for performance improvement
* Analyzes trends in LOS, census, readmissions, and non-qualified hospital days and the impact on continuum-based utilization and outcome
* Coordinates efforts to optimize hospital days, including ineffective admission determinations, discharge planning, utilization of ancillary services and timely placement
* Works closely with the Capacity Center to identify opportunities to enhance throughput efforts and optimize discharge options
* RN with current license to practice in Massachusetts required.
* Bachelor's degree required; master's degree preferred.
* Case Manager certificate preferred.
* 5 years of inpatient case management experience with advanced knowledge around standards of practice and regulatory and payer compliance required.
* Extensive experience and knowledge in discharge planning required.
* Extensive understanding of post-acute resources required.
* Utilization Review experience preferred.
* Case Management in acute setting and Utilization Management experience/knowledge
* Strong assessment and problem-solving skills
* Strong interpersonal skills
* Ability to work independently with minimal supervision.
* Goal oriented and accountable
* Demonstrated organizational skills.
* Demonstrated ability to work in a complex setting.
* Ability to work in an interdisciplinary team-based environment.
* Strong oral and written communication skills
* CM Certification preferred.
* Computer experience preferred.
