Job Description

Position Summary:

This individual will be accountable for reducing the re-admission rate for a defined high-risk population in the Northwest Region. This accountability will include providing clinical services in the home and clinic settings which effectively reduce preventable re-admissions in the aforementioned population. This includes providing direct patient care, as well as triaging to appropriate coordination, social and behavioral health resources. The role includes the NP taking full-responsibility for the patient from post-discharge to sustainable stabilization-not at high-risk risk for subsequent re-admission. There will be a strong focus on co-morbid CHF/COPD patients.

Essential Duties and Responsibilities include the following:

  • Provides care to defined high-risk members, either in the home or at the clinic
  • Adheres to evidenced based clinic guidelines
  • Appropriately documents, per Regal and regulatory compliance rules, all care provisioned
  • Ensures appropriate follow-up for patient including social and behavioral services
  • Tracks clinical/social issues, patients seen
  • All visits must be conducted timely, and adhere to group/health plan guidelines and protocols.
  • All other duties as assigned.

The pay range for this position at commencement of employment is expected to be between $125,000 - $130,000 annually; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, licensure, skills, and experience.

The total compensation package for this position may also include other elements, including a sign-on bonus and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered.

If hired, employee will be in an "at-will position" and the Company reserves the right to modify base salary (as well as any other discretionary payment or compensation program) at any time, including for reasons related to individual performance, Company or individual department/team performance, and market factors.

Requirements

Education and/or Experience:

  • Current Certification as Nurse Practitioner
  • Experience with COPD/CHF specific care
  • Experience with high-risk, complex patients
  • Experience delivering care in home-settings preferred but not required
  • Minimum 3-4 years experience in health care
  • Excellent verbal and written communication skills in English
  • Proficient at Microsoft Office products
  • Ability to work independently in the field, while still meeting team deadlines and requirements
  • Bi-lingual (Spanish) preferred but not required
