Job Details

Catholic Health Home Care's Quality Improvement Specialist RN plays a crucial role in ensuring the delivery of high-quality care within the agency. This position involves collaborating with various departments, analyzing data, and promoting effective quality care practices. The specialist also works closely with interdisciplinary teams, advocating for quality improvement initiatives and providing support in documentation and performance tracking. Reporting to the Director of Performance Improvement, this role requires a blend of clinical expertise, analytical skills, and effective communication.

Quality Improvement Specialist RN

* Collaborates with the Performance Improvement Department and Clinical Operations in tracking agency performance.
* Responsible for gathering, compiling, analyzing data and producing information to promote effective Quality Care.
* Partner with an interdisciplinary teams within the agency.
* Advocate for and builds relationships with interdisciplinary team.
* Presents and reviews monthly performance scorecard.
* Participate in safety initiatives at the team and agency level to improve OASIS outcomes.
* Provide real time accountability and coaching related to OASIS documentation and Home Health Value Based Purchasing.
* Participates in Agency Quality Meetings
* Reports to Director of Performance Improvement

What you'll bring:

* Passion for Quality Improvement and Education
* Experience in Home Health Care
* Experience in Quality/Performance Improvement preferred
* Excellent communication and public speaking skills
* Skilled in the use of electronic medical record EPIC, MS Excel, MS Office, & MS PowerPoint
* Education: Graduate of an accredited School of Nursing required
* Bachelor of Science in Nursing (BSN) degree required with a Master of Science in Nursing (MSN) degree preferred
* Licensure: New York Registered Nurse (RN) License & Registration

Salary Range

USD $90,000.00 - USD $113,000.00 /Yr.

This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate's qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
