Job Summary/Description:


Responsibilities / Essential Job Functions:

  • Using Motivational Interviewing and a collaborative approach, conduct in-depth assessments to best determine the immediate client concerns and/or barriers to housing including, but not limited to: financial, legal, mental health, substance abuse, employment, health care and housekeeping
  • In collaboration with heads of households, develop a Re-Housing Plan (RHP) for each family member over 18 years of age. The (RHP) must include specific goals in the areas of housing, education and training, financial management, employment and career development. The Case Manager is expected to meet with families, at least bi-weekly, to review (RHP) progress and plan next step
  • Document and track all families’ meetings, interactions and information in the ETO (Efforts To Outcomes) database. This includes, but is not limited to, HUD Assessments, RHP, Case Notes, Validations and Clients Lists
  • Manage a case load of 15 to 19 families
  • Complete a monthly unit inspection for each family assigned
  • Complete quarterly client file audit
  • Serve as liaison and collaborate with programs, making sure families fully participate and benefit from these programs’ services. The Case Manager is expected to successfully refer all new families to the Economic Mobility Center and Placement Program for their initial assessments
  • Obtain resources and services as requested and/or needed to address barriers to housing as stated above including but not limited to; CORI mitigation, credit repair, and assistance with financial applications
  • Ensure that all clients receive explanation and assistance with navigating housing application processes and referral to all appropriate housing opportunities; including but not limited to, local housing authorities, regional non-profit housing agencies, private management companies, community development corporations, among others
  • Develop partnerships and collaborative relationships with internal and external service providers to ensure clients receive comprehensive services
  • Participate in the on-call weekly rotation schedule (Required)
  • Participate in individual and group supervision as assigned by Program Manager
  • Participate in all required training and professional development activities
  • Work collaboratively with Development staff to participate in volunteer activities as needed
  • Perform other tasks as requested by Program Manager, Assistant Director of Family Services and/or Director of Family Services
  • Observe all program and agency rules and policies
  • Other duties as assigned by the Program Manager
  • Perform other duties as assigned.


Qualifications / Requirements: (education, years of experience, knowledge base, skills)

  • Bachelor’s Degree in Social Work (or related field) plus two years related work experience; Any combination of education & experience that equips candidate to perform above mentioned tasks
  • MSW (or related filed) degree preferred
  • Bilingual in Spanish
  • Case management experience
  • Experience with crisis intervention regarding issues such as homelessness, mental health, substance abuse and other housing barriers
  • Valid MA driver’s license
