Job Description Summary:

The CNA assists with hands-on patient care in accordance with the Care Plan, and assists in maintaining a safe and clean environment under the direction and supervision of a Registered Nurse.

Essential Job Functions/Responsibilities:

A. Assists in maintaining a clean and healthy environment for the patient.

B. Provides hands-on personal care in accordance with the care plan.

C. Observes and reports significant changes in the patient's status to the Registered Nurse.

D. Encourages the patient to achieve maximum level of self-reliance.

E. Confers with Hospice Care team members to coordinate patient care.

F. Maintains patient confidentiality at all times.

G. Follows patient care instructions in the home.

H. Assists in ambulation or exercise.

I. Assists in follow-up patient activities and conditions as instructed by supervising nurse.

J. Completes patient records in a timely manner.

K. Reports (and when appropriate, records) any changes observed in condition or behavior of patients and unusual incidents.

L. Participates in in-service education programs, as assigned, to learn new treatments, procedures, developmental skills, etc.

M. Participates in after hours, weekend, and holiday rotation.

N. Establishes and maintains interpersonal relationships with patients and family while ensuring confidentiality of patient information.

O. Provides own transportation in good working order, with appropriate levels of insurance.

P. Knowledgeable and accepting of age appropriate-cultural/ethnic differences of patient/family served.

Q. Performs all other duties, as assigned

R. Demonstrates exceptional service to patients, families, referral sources and co-workers and other customers.

Position Qualifications:

1.CNA with current certification required

2.Hospice experience preferred.

3.Exhibits caring attitude toward patients and families; maturity and ability to cope with stressful situations.

4.Travel required so reliable transportation and current auto insurance is necessary
