A Home Health Aide is a trained, Certified Nurse Aide who is prepared to assist professional nursing personnel in the skilled aide care of the patient in the home. Will function as a substitute for a family member in providing home nursing care on a part-time/intermittent basis for the sick and/or disabled member of the family. The duties of the Home Health CNA are planned and assigned by the professional nurse in conjunction with the physician to meet patient and family needs. Care assignment times may vary according to patient needs.
Position Responsibilities :
- Under the direct supervision of a licensed nurse, carries out quality patient care according to hospital policies and procedures, functioning within the limitations of job scope.
- Provide personal aspects of care to patients to increase the level of comfort and maintain personal hygiene.
- Provide temporary respite to the patient's family and provide relief from total personal hygiene.
- Assist in maintaining a safe, healthy environment for the patient and family.
- Report significant changes in the patient's condition to the registered nurse for follow through and appropriate intervention.
- Carry out Homemaker/Home Health Aide tasks which can be delegated and supervised by appropriate health professionals and have been reviewed by the Colorado Board of Nursing.
- Assists staff nurse with physical care of patients.
- Maintains skill level required to care for routine patients.
- Assures that Patient/Family needs are met.
- Maintains timely documentation.
Requirements
- Completion of a state certified training program.
- CNA required or eligible within 60 days or FT student enrolled in RN or LPN program. Transcripts must be on file.
- At least one year's relevant experience preferred.
- BLS required or eligible within 60 days.
Skills and Special Abilities :
- Ability to function independently in the home environment. Sound judgment in decision-making; can react quickly and with confidence in a crisis or emergency situation.
- Recognition of own limitations in nursing practice and ability to seek out support and expertise of others.
- Compassionate, patient approach in working with chronically ill elderly.
- Adequate awareness of the dynamics of human behavior, especially the results of illness/disability on the family unit.
- An understanding of some of the special problems facing the elderly, the terminally ill or the acutely or chronically ill child and an ability to mobilize resources that will facilitate an optimal home environment.
- A non-judgmental, accepting attitude towards varying lifestyles and living situations.
- Knowledge of community resources and utilization of community services to augment services provided by Delta County Memorial Hospital.
- Able to care for patients of all ages.
- Ability to work effectively within role independently and with other team members.
- Ability to organize and complete work in a timely manner.
- Ability to read, write and effectively communicate in English.
Other :
- Must maintain an active driver's license.
- Must maintain a personal vehicle in operational order for travel in varying weather conditions at varying times of the day or night.
- Must maintain current proof of automobile/vehicle insurance.
- Must maintain an operational phone or equivalent means to be contacted except when on leave or vacation.
