Job Summary

The Home Health Registered Nurse maintains high standards of professional nursing practice in the home care setting under the guidance of the Home Health Administrator. Key responsibilities include assessing and providing care within their scope of practice, adhering to local, state, and federal guidelines, and collaborating with the healthcare team to develop effective patient care plans.


Responsibilities:
  • Deliver comprehensive nursing care, taking into account age and developmental needs throughout all phases of care.
  • Collaborate and support plan of care as determined by the physician and healthcare team; treat patients accordingly as approved by the physician.
  • Perform periodic re-evaluation of the patient as necessary and make adjustments in the patient's treatment program.
  • Provide treatment to patients per protocols, utilizing OASIS assessment and documentation.
  • Conduct on-going periodic system assessment and data collection in a systematic manner, focusing on psychological, psychosocial, cultural spiritual and cognitive status; nutrition; pain; patient/family education; family involvement and patient advocacy; patient and patient's equipment needs.
  • Facilitate timely discharge based on goals and in accordance with policies and procedures.
  • Educate and instruct the patient, patient's family, and other staff as required.
  • Counsel the patient and family in meeting nursing and related needs.
  • Supervise and teach LPN's, HHA's as required for patient care according to law and regulations.
  • Document all care, intervention, and care coordination per law, regulation, and agency policy.
  • Notify patient's physician and responsible staff of significant changes in a timely manner.
  • Coordinate all services provided; consult with other health professionals as needed.
Requirements:
  • Graduate from an accredited school of nursing; BSN preferred.
  • Current license to practice as a Registered Nurse in Colorado.
  • A minimum of two (2) years' experience as a Registered Nurse.
  • Previous Home Health experience is preferred.
  • Clinical expertise in critical thinking and physical assessment of both the adult and children.
  • BLS certification required.

Compensation Package

Our estimated salary range for this position is $85,000-$105,000 annually, depending on qualifications and experience. This figure takes into account industry standards, regional factors, and the level of responsibility associated with this role.
