Major Tasks, Key Responsibilities and Key Accountabilities
— 1 Completes an initial assessment of patient and family to determine home care needs.
— 2 Regularly reevaluates patient nursing needs
— 3 Initiates the plan of care and makes necessary revisions as patient status and needs change
— 4 Uses health assessment data to determine nursing diagnosis
— 5 Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions.
— 6 Includes the patient and the family in the planning process
— 7 Initiates appropriate preventive and rehabilitative nursing procedures.
- 8 Administers medications and treatments as prescribed by the physician
Job Overview and Requirement
Job Type: Registered Nurse-Home Health
Qualifications:
Graduate of an accredited school of nursing. One to Two years of recent acute care experience in an institutional setting
Current RN license in the State of Colorado
CPR Certified
Bachelor's Degree, with one year of home health care experience preferred.
Excellent observation, verbal and written communication skills, problem solving skills, basic math skills, nursing skills, etc.
Reliable transportation and license to drive.
