Role Summary: Are you a compassionate and skilled Registered Nurse looking for a role where you can make a real impact? Our client is seeking dedicated nurses to join their team, providing exceptional care both in the clinic and in patients' homes. This position offers a blend of clinical care, planning, and implementation, allowing you to utilize your nursing expertise to its fullest. You'll be part of a supportive environment where your contributions are valued and your professional growth is encouraged.
Essential Duties and Responsibilities:
Assessment:
- Collect patient data and complete required forms according to department and industry standards.
- Identify patient needs and prioritize accordingly.
- Perform advanced nursing observations and critical thinking assessments.
Planning Care:
- Develop care plans based on nursing processes and incorporate plans from other disciplines.
- Include patients and families in developing or revising care plans.
- Complete initial and periodic assessments, communicating participant changes to team members.
Implementing/Managing Care:
- Ensure care conforms to accepted nursing practice standards.
- Provide correct treatments and procedures as prescribed.
- Educate participants and families, coordinate 24-hour care delivery, and manage emergency situations.
- Document all nursing treatments accurately and timely.
Evaluation:
- Record patient care delivered and any variations with appropriate rationale.
- Make and document observations and implement nursing measures for impending or associated problems.
Leadership:
- Delegate and supervise direct patient care.
- Integrate cost-effective measures into nursing practice.
- Participate in QI processes and comply with policies for safe working conditions.
Interdisciplinary Team:
- Actively participate as a cooperative member of the Interdisciplinary Team.
- Attend staff meetings and contribute to participant care planning.
Other Duties:
- Perform other projects and duties as assigned.
Requirements:
- Graduate of an accredited school of nursing.
- Current RN license in good standing to practice in the required state.
- Minimum 3 years of RN clinical experience within the last 3 years.
- Experience with paperless documentation preferred.
- 1 year of experience working with a frail or elderly population preferred (training provided if not present).
- Reliable transportation, valid driver's license, and minimum state-required liability auto insurance.
- Medical clearance for communicable diseases and up-to-date immunizations.
- Comprehensive criminal background check.
Skills and Abilities:
- Effective teamwork and communication skills.
- Knowledge of clinical nursing treatment for geriatric patients.
- Basic computer skills and email usage.
- Ability to meet standardized competencies before working independently.
Work Environment/Demands:
- Physical demands include standing, bending, walking, sitting, typing, talking, and hearing.
- Work within an interdisciplinary team model in an Adult Day Health Center and primary care clinic environment.
