As an SMI Primary Care Nurse Practitioner, you will play a crucial role in providing comprehensive primary care services to patients with Serious Mental Illness. You will be responsible for conducting physical exams, diagnosing and treating common illnesses, prescribing medications, and providing health education and counseling tailored to the unique needs of SMI patients.

Job Responsibilities:

  • Provide primary care services to patients with Serious Mental Illness (SMI)

  • Collaborate with mental health professionals to ensure coordinated care for SMI patients
  • Monitor and manage chronic conditions in SMI patients
  • Document patient encounters and maintain accurate medical records
  • Ensure compliance with healthcare regulations and standards
  • Participate in quality improvement initiatives and professional development activities

Requirements

  • Valid Nurse Practitioner license and certification
  • Master's degree in Nursing from an accredited program
  • Excellent clinical and diagnostic skills
  • Strong communication and interpersonal skills
  • Ability to work effectively with a multidisciplinary team
  • Knowledge of mental health interventions and evidence-based practices
