Salary Rate: $165,000/yr

Job Summary

Responsibilities:

  • Perform a whole-person clinical assessment
  • Gathering information from outside sources (especially other care providers)
  • Assessing both behavioral and physical health conditions through a thorough history and physical examination
  • Capturing conditions not yet diagnosed, including by ordering additional studies as indicated
  • Organizing the information in a manner that supports successful disability applications (if appropriate)
  • Conducting visits in the field or in an individuals home when needed
  • Making individualized recommendations and/or referrals for programs/care provided by Client or outside providers, based on the assessed need.

Experience:

  • 5+ years clinical care as a Nurse Practitioner, ideally in a primary care setting
  • Working with and managing underserved populations with challenges such as behavioral health and/or SUD conditions
  • Developing and delivering educational programs to nurses and affiliated medical staff
  • Care management and coordination firsthand Benefits

Preferred:

  • Master's degree or higher
  • 3+ years of experience in ANY of the following:
    • Primary Care
    • Value Based Care
  • Valid Nurse Practitioner (NP) in the state of Washington (WA)
  • Home visits are a major component of this job. Are you okay visiting patients in their homes? (yes)
