Restore First Health is hiring for a Wound Care RN for our New Jersey practice. The Wound Care RN manages, organizes, and provides follow up care in collaboration with the Nurse Practitioners for Restore First Health in accordance with applicable federal, state, regulatory, and company standards to ensure the delivery of quality patient care is rendered at all times, and that the Plan of Care and physician orders are followed with regard to established clinical protocols. This position will be responsible for continuity of care post Primary Wound Therapy Protocol Application sequence. This position demonstrates above average communication skills, personal integrity, and works effectively with patients, family members, physicians, staff, and outside agencies as required. The position works in conjunction with the Restore First Health’s Advanced Wound Therapy program education efforts, as well as educates patients and family members on Advanced Wound Therapy and its interplay with current conventional wound care. This position requires continuous updating of patient care, medical records transfer and procuring of patient records, and other care coordination as directed.

In consultation with the Medical Director and Nurse Practitioners, assists in the development of appropriate plans of care for comprehensive wound management.

Participates in business development activities to promote Restore First Health facilities as the provider of choice for Advanced Wound Therapy.

Involved in market establishment of the wound care program through outcomes and relationships with physicians and other various healthcare agencies such as home health companies, skilled nursing, and assisted nursing facilities.

Care Coordination, Quality Assurance, and Scheduling

Upon completion of 12-week Primary Wound Therapy Protocol, reviews and evaluates continued wound care needs via scheduled visits to the patients for follow-up and addressing any issues. Establishes continued relationships with providers, families and patients through documented care plans.

Reviews charts of patients with wounds. Ensures that wound assessments, care plans, and treatments are clearly and correctly documented and that appropriate wound related treatments are being provided. Investigates all cases with adverse events related to wounds.

Patient and Provider Education

Serves as a consultant to facility staff and advocates with physicians; through training and support, enables clinical staff to effectively assess wounds, recommend appropriate protocols, and initiate plans of care.

Educates staff on products available for use in the program. Educates families on care of patient(s), and potential referring physicians.

Works closely with the Director of Operations to educate and report updates with respect to discharge procedures, medical records transfer, and scheduling post allograft treatment visits.
