Dynamic Mountain View social services agency seeks a full-time Licensed Vocational Nurse (LVN), Case Manager. Hours are usually Monday - Friday, 8:00a.m. - 5:00 p.m.; however the position does require some evening and weekend hours (40 hours per week).

Responsibilities:

Case management services to frail elderly clients with chronic health conditions, which includes coordination of post-hospital discharge appointments with primary care physicians ,blood pressure monitoring, provision of chronic disease management educational materials, coordination of support services and link clients to community resources to help them better manage their chronic health conditions and avoid hospitalization/readmission to the hospital.

Requirements:

Current California License and graduate of an approved LVN School of Nursing and a minimum of two years of nursing experience working with older adults. Ability to handle multiple projects, meet deadlines, manage priorities and workflow while maintaining a positive, patient and calm demeanor. Excellent interpersonal and listening skills and ability to exercise excellent judgement, to take initiative and make thoughtful and timely decisions. Must possess CPR certification.

Compensation & Benefits:
