Your Role as a Home Health Occupational Therapist: Provide occupational therapy to patients who struggle with daily tasks due to illness or injury. Collaborate with a home health team potentially including an RN, LPN, CNA, HHA, PT, and SLP, focusing on patient care and personal goals. Assess patients, observe deficits, and establish therapy goals while documenting progress. Assist patients with exercises to enhance fine motor skills and coordination. Recommend adaptive equipment such as grab bars and shower chairs to provide additional support. Identify fall risks and implement strategies to improve home safety. Educate patients and their families on the plan of care, exercises, goals, and self-care. Qualifications for Home Health Occupational Therapists: Current licensure or certification as an Occupational Therapist (OT) in the state of Arizona. Graduate of an accredited Occupational Therapist program. CPR Certification (demonstration course required). Valid Arizona Fingerprint Clearance Card. Experience in home health preferred.
