About the Opportunity:
Nurse Practitioner | Physician Assistant
Schedule: M-F 8am-5pm
Location: North Carolina - Fayetteville, Raleigh, Durham, Winston, Salem, Charlotte
Patient load: 6-8 on average

Responsibilities:

- Provide compassionate, holistic patient care, addressing diverse needs and collaborating with multidisciplinary teams.
- Deliver exceptional clinical care in-person and virtually, with in-person settings including private homes, congregate living facilities, and/or other home settings.
- Integrate practices and processes to achieve company goals.
- Maintain accurate medical records and participate in initiatives for patient engagement.
- Collaborate with external providers and partners.

Qualifications:

  • NP or PA with graduation from an accredited health professions school and successful completion of national board certification.
  • Active licensure in applicable state(s).
  • Active DEA licensure and state-specific controlled substance registration, as applicable.
  • BLS required.
  • Clinical experience caring for aging adults and adults with complex care needs required.
  • Ability and means to travel as needed in a timely manner to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required. DRIVER REQUIREMENTS: Licensed for a minimum of 5 years. No vehicle-related suspensions/reinstatements, DUI, reckless driving, or felony convictions within the last 7 years.
  • Must be fully vaccinated for Covid
