Responsibilities/General Duties (including but not limited to):
  • Provides direct patient nursing care to Hospice residents and provides on-site responsibility for nursing assistants, working closely with the staff and volunteers
  • Contributes as a team member to identify needs for patient plan of care.
  • Reports change in resident's condition to RN Case Manager, Team Leader, and/or communicates to resident's physician or Hospice Medical Director
  • Documents appropriately on patient records
  • Assists with food preparation for Hospice residents when necessary
  • Assists with necessary housekeeping to maintain quality environment for residents at all times
  • Assists House Manager in maintaining health, fire and safety measurements according to policy and procedure
  • Assures utilization of universal precaution for infection control at all times
  • Assists Team Leader in conducting clinical observation of CNAs and other house staff in preparing written evaluations
  • Provides guidance and direction to employees reporting to him/her. Ensures staff receives the needed training. Reviews and adjusts staff work duties as required
  • Proactively supports the Individual Achievement Review and Evaluation (IAR&E) performance evaluation process. Communicates with staff and provides frequent coaching and feedback to enhance individual performance
  • Follows HOP personnel policies and procedures (i.e., recruitment and hiring process, completion of accident reports, etc.)
  • Completes other assignments and tasks as requested
  • Responsible for obtaining medications via Hospice Pharmacia and or local pharmacy on admission and any new orders while on duty. Performs narcotic count with shift change, adheres to policy/procedure for safekeeping of all medications, and the destruction of medications with proper documentation

Typical Physical Demands/Working Conditions:
  • The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of the job.
  • While performing the duties of the job, the employee is regularly required to use hands to handle, feel objects, tools or controls, reach with hands and arms; stoop, kneel, crouch, or crawl; and talk to hear. The employee frequently is required to stand, sit and climb or balance.
  • The employee must occasionally lift and/or move up to 50 pounds and push up to 200 pounds in a wheelchair. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and the ability to adjust focus.
  • Incumbent must be able to communicate by way of telephone. Keyboarding is required. Regular use of computer screen is required to complete tasks. Ability to drive and to negotiate steps.
  • Noise level is low. Physical and emotional health necessary for the accomplishment of position duties.

Requirements

Qualifications:
  • High School Graduate or GED
  • Graduation from an accredited school of practical nursing
  • Licensed as a Licensed Practical Nurse in the Commonwealth of Virginia
  • Two years relevant nursing experience required
  • Ability to manage all aspects of terminal care, health team service coordination, and documentation
  • Nursing home/assisted living facility experience preferred
  • Basic knowledge of Medicare/Medicaid reimbursement regulations
  • Emotionally mature and caring individual willing to give support and assistance to terminally ill and their families
  • Basic to intermediate proficiency in word processing software helpful
  • Ability to effectively present information in one-on-one and small group situations to clients and other employees of the organization
  • Ability to write reports, correctly reflect treatment and document client progress
  • Good oral and written communication skills
  • Skill in establishing and maintaining effective working relationships
  • Must be flexible and able to function in stressful situations
  • Incumbent should have the ability to manage multiple tasks
  • Ability to effectively communicate and work with family members, staff and management
  • Ability to work with and maintain confidentiality of patient data
  • The ability to work as a team member is necessary for this position.
  • Due to the nature of services provided, it may be necessary at times for employees to work beyond scheduled work hours to ensure adequate patient care
  • Must demonstrate basic computer skills.
