We are seeking an experienced Registered Nurse to join our team as an Inpatient Hospice Nurse Leader. As a key member of our interdisciplinary team, you will be responsible for planning, organizing, and directing inpatient hospice care in our Center for Acute Hospice Care and Hospice House.

About the Role

In this role, you will work closely with our Clinical Services Manager to ensure that patients receive high-quality, person-centered care. You will also be responsible for communicating effectively with patients, families, and healthcare professionals to ensure seamless transitions of care.

Key Responsibilities:
  • Patient Care:
  • Provide professional nursing care to patients and their families in accordance with the plan of care and organizational policies and procedures.
  • Assess and evaluate patient status, regularly re-evaluating needs and revising the plan of care as necessary.
  • Develop and implement individualized plans of care that address the unique needs of each patient.
Requirements:
  • Graduate of an accredited nursing school.
  • Licensed as a Registered Nurse (RN) in the Commonwealth of Virginia.
  • Minimum of 2 years hospice, home health experience, ICU CCU experience.

We offer a competitive salary range of $65,000 - $85,000 per year, depending on experience, and a comprehensive benefits package.
