The Medication Therapy Management Specialist focuses on optimizing medication therapy for patients through comprehensive reviews education and collaboration with healthcare teams.

Key Responsibilities:

  • Conduct thorough medication reviews and essments to identify potential drugrelated problems.
  • Develop and implement individualized medication therapy plans in collaboration with patients and healthcare providers.
  • Provide patient education on medication usage adherence and lifestyle modifications.
  • Monitor patient progress and adjust therapy based on clinical outcomes.
  • Doent and report medicationrelated interventions and outcomes.

Requirements
  • Doctor of Pharmacy (Pharm.D.) degree with training in medication therapy management.
  • Board certification in medication therapy management (BCMTMS or equivalent) preferred.
  • Strong ytical and clinical ss.
  • Effective communication and interpersonal ss for patient and provider interactions
