Job Description

*Description:* The Physical Therapist Assistant assists the Licensed Physical Therapist in providing quality Physical Therapy services including patient treatment under the direction and supervision of a licensed Physical Therapist in a home health environment.

This per-diem position will support Home Health, looking for someone with flexibility to cover Beaver County.

*Responsibilities:*

* Provides Physical Therapy (PT) treatment to patients according to the Physical Therapy plan of care as directed and supervised by the licensed Physical Therapist.
* Participates in gathering information regarding functional status of patients as directed by the licensed Physical Therapist.
* Completes all documentation and record keeping related to the provision of Physical Therapy services in a timely and accurate and thorough manner consistent with regulatory requirements.
* Participates in the interdisciplinary care plan process and communicates to patients, families, caregivers and facility/agency staff regarding care issues and discharge planning.
* Participates in the operation of the Rehabilitation department according to department policies and procedures, i.e., infection control, safety.
* Complies with all facility/agency policies & procedures and facility/agency staff regarding care issues and discharge planning.
* Contributes to the planning, implementation, evaluation and quality management of clinical, department and facility/agency programs.
* Attends Rehab department meetings and participates in all mandatory in-service programs specific to the Rehab department.
* Provides appropriate caregiver training as requested.
* Complies with Pennsylvania Physical Therapy Practice Act and all federal and state regulations applicable to Pennsylvania.
* Performs all other duties as assigned. \*
* Ability to work independently and within a multidisciplinary team. Valid driver’s license necessaryAll qualified applicants will receive consideration for employment without regard to race, color, religion, gender, national origin, age, disability or veteran status.

*Qualifications:*

* Associate's degree (A.A.) from an accredited Physical Therapist Assistant program.
* Two year related experience and/or training preferred.
* Working knowledge of computer software required for documentation and communication.
* Maintains current PA License as a Physical Therapist Assistant, including meeting continuing education requirements.
