Position Overview:

• Position Title: Resource Specialist

• Location: Remote, with fieldwork in regions such as New Port Richey, Clearwater, Tampa, Orlando, Maitland, Sarasota, and Fort Myers.

• Target Start Date: October 14 and October 28

• Reason for Opening: Expansion due to Sunshine Health's recent award of the sole source health plan for the child welfare population.

• Employment Type: Full-Time, with a potential for extension.

• Salary: $60,000 (FTE conversion).


Job Responsibilities: Field-based care management: Includes home visits, group home/hospital settings, and attending community/agency meetings.


Caseload: Manage a caseload of 100 members (children and young adults).


Core Tasks:

• Assess healthcare needs.

• Coordinate care with health plan providers.

• Provide education on health benefits.

• Connect members with community resources to address social determinants of health.

• Document outreach, assessments, and care plans in an electronic health record system.


Work Hours:

• Schedule: Monday to Friday, 8 AM - 5 PM.

• Additional Time: Weekend work may be needed during hurricane season for disaster outreach.


Performance Metrics:

• Compliance with care management policies and procedures.

• Monthly audits to ensure documentation and charts are compliant.


Required Skills & Experience:

• Pediatric case management experience.

• Experience connecting families/children to local resources (employment, housing, independent living, foster care, justice services, etc.).


Preferred:

• Health plan experience.

• Foster care and/or adoption experience.


Education Requirements:

• Required: Bachelor’s Degree


Software Skills: • Proficiency in Microsoft Office: Outlook, Word, and Excel.


Interview Process:

• Interview Format: Panel interviews, 30 minutes each, with two interviewers.

• Preferred: Interview Blitz—conduct all interviews in one day (9 AM - 5 PM), with a 1-hour lunch break.
