Hospice RN - Night Shift

Come join a team of seasoned nurses at Care Partners, one of Oregon’s oldest not-for-profit hospices For over 40 years, we have cared for hospice patients across the Portland Metropolitan area, bringing interdisciplinary and holistic hospice services to patients and their support systems. Care Partners is seeking a RN with previous experience in the Hospice field, ICU, med-surge or ER. This position has candidates in the field providing direct patient care. The onboarding and training process will involve hands-on learning with an RN preceptor, directly supported by our Director of Nursing. Having hospice experience is a must

Schedule Nights and weekends, scheduled varied and flexible.

Your Responsibilities (Includes but Not Limited to):

Provide visits and advise by telephone to patients as needed to address symptom management and other needs that arise in the evening or during the night.
Provide patient care to patients across settings in private home, adult foster home, residential/assisted living care, and/or nursing facility following existing care plans.
Contribute to patient assessment under the supervision of a registered nurse by assisting in identifying changes in patient status
Obtain necessary physician orders and collaborate with RN and attending physician on patient needs and follow up on and obtain lab results and notify attending physician of results
Monitor signs and symptoms as well as reactions to interventions, treatments, and medications under the direction of the RN
May initiate medication changes while working with physician to get medication orders
Utilize appropriate standards of care for the identified medical diagnosis
Collect, document and report to the Registered Nurse (RN) when necessary, both objective and subjective data regarding patient status
Utilize Universal Precautions and safety measures to ensure a safe care environment for patients including consistent use of personal protective equipment (PPE)
Demonstrate competencies and knowledge related in the standards of practice for a licensed practical nurse in the state of Oregon when providing care
Utilize and become proficient in navigating the electronic medical records system for documenting and reviewing patient conditions and plan of care
Demonstrate an understanding and respect for patient rights when providing care
Establish and maintain effective working relationships with the patients and families, co-workers, and members of diverse cultural backgrounds. Know and apply principles of an inclusive work environment.
Complete annual mandatory education review and competency programs

Qualifications:

Graduate of an accredited nursing program from school nursing
Current unencumbered Oregon RN license prior to the first day of employment.
Requires at least one year of experience in nursing; long term care, acute care, med-surge, ICU, ED, ambulatory care or home health and hospice care recommended and demonstrated knowledge in thorough clinical documentation.
At least one year of experience in hospice care recommended.
Must have current Basic Life Support CPR (AHA-BLS or ACLS) card for Health Care Provider. Valid card must be obtained prior to patient contact and within two weeks of hire
Must have a valid driver’s license and acceptable driving record and have access to a reliable automobile to travel to and from patient homes.
Experience working independently with minimal supervision while delivering effective and on-time results.
Strong organization skills, time management and attention to detail.
Excellent oral, written, and interpersonal communication skills.
Must be reliable and dependable in completing work on time, following through to complete tasks, and be available for scheduled hours.
Bi-lingual, Spanish speaking ability and certification in Hospice and Palliative Care are helpful

*Preferred applicant will be asked to pass a background check and drug screen following a job offer.

**New employees are required to complete tuberculosis screening and provide proof of Hepatitis B vaccination or signed declination.
