Job SummaryWe are seeking a highly skilled and compassionate Registered Nurse to join our team. As a Registered Nurse, you will play a vital role in delivering high-quality patient care and contributing to the growth and success of our organization.
Key Responsibilities- Conduct thorough assessments of patients' physical, functional, spiritual, psycho-social, and environmental needs.
- Develop and implement individualized care plans in collaboration with the interdisciplinary team.
- Provide education and support to patients and their families on disease management, self-care techniques, and end-of-life care.
- Communicate effectively with the medical director and other clinicians to ensure seamless care coordination.
- Maintain accurate and detailed documentation of patient observations, findings, and services provided.
