We strongly encourage people of all backgrounds to apply.Supervisor: Health Care Partnerships Program ManagerSalary Range: $56,000-$59,000The Role: Project Bread is seeking a collaborative, detail-oriented, and compassionate professional to join our Health Care Partnerships team. The Program Associate plays an essential role turning client care plans into meaningful resources, coordinating logistics between case managers and clients, to help ensure clients receive the resources and nutrition education they need to prepare and eat healthy foods.Because Health-Related Social Needs Services is a new program, we're looking for an Associate that can adapt quickly, stay organized amid change, and respond to clients and colleagues with empathy and clear communication. The ideal candidate enjoys both meaningful client interactions and high-impact operations work, and is comfortable learning new technologies. Spanish-speaking candidates are strongly encouraged to apply.About Health-Related Social Needs ServicesThe Commonwealth of Massachusetts received a waiver from the Centers for Medicaid and Medicare to improve the ways in which MassHealth provides care to its members, which includes supporting health-related nutrition and housing needs as part of their strategy to address social determinants of health.Project Bread is an approved nutrition service provider. In this role we receive referrals and provide nutritional services to members from over 45 health centers, hospitals, and physician offices across Massachusetts.Primary Responsibilities Fulfill orders for kitchen supplies and appliances. Coordinate the delivery of goods and services to clients by working with their coordinators to ensure lost items or returned mail arrive in the hands of clients. Provide daily customer service support to individuals enrolled in the program by answering phone calls, emails, and voicemails. Support cooking classes by scheduling, managing translation services, and sending reminders to clients. Support program nutrition education work through managing document translation and general organization. Assist with record keeping and reporting as needed. On occasion, complete outreach directly to members for program enrollment and service support needs. Perform other related tasks as needed.Organizational Responsibilities Participate in the annual Walk for Hunger, including working as event staff the weekend of the Walk (first weekend in May), and attending pre-Walk events as assigned. Participate in organization-wide events and meetings. Database experience (Salesforce preferred). Fluency in Spanish is strongly preferred. Prior professional experience, preferably in customer service or public service.Work ScheduleAll employees are currently working a hybrid remote and in-office model and are expected to be present in the East Boston office on a regular basis. Remote work schedules are determined by departments. The Program Associate is required to work from the office two days a week, including full-team workdays on Wednesdays.
