Our client is hiring an LPN with 1 year of experience and some home care experience.

Job Function:

Administers and monitors the effectiveness of medications/treatments ordered by physician and documents same.
Assists in implementing the care plan by performing patient’s care as based upon identified needs consistent with plan of care and mutually agreed upon goals with patient/family/Case Manager.
Evaluates the effectiveness of the plan of care and notifies Case Manager and Physician of changes in patient care.
Responds to emergencies and reports to the Registered Nurse and Team Manager.

Job Requirements:

NJ LPN License
1 year of experience
Home Care experience required
OASIS experience highly preferred
